Name the avoidable mistake and let the hand over the face supply the response. This is the moment after someone realizes how unnecessary the problem was.
The facepalm can belong to the person who made the mistake or to someone watching it happen. Tell the reader whose embarrassment or frustration they are seeing.
The covered eyes give the reaction a tired quality. Use it for an error you recognize immediately, especially one that has happened before.
Write the event in one sentence at the bottom of the frame. Keep the hand and forehead clear; those are more important than adding a second caption.
You can make the joke about yourself. Forgetting an attachment or looking for an item already in your hand gives the reaction a clear cause without needing a long story.
